You are a rival Victorian doctor competing for patients at an asylum. Claim lunatics, commit treatment cards, and pray the surgery goes well. The doctor who commits the fewest cards wins the patient — but rolls dice for every missing requirement. Cure patients for points. Kill them for shame.
Each round has 5 phases:
Click patients in the Ward to claim them. AI opponents have already made their claims. Click "Done Claiming" when ready.
When multiple doctors claim the same patient, a descending auction begins:
Reveal your committed cards and match colors to the patient's requirements:
Face cards fill their suit's color requirement AND activate a special ability:
| Ace | The Lobotomy | Matches suit color. If surgery fails, saves the patient — but base points become 0. Still counts as cured. |
| Jack | Laudanum | Matches suit color. All dice succeed on 2+ instead of 4+. |
| Queen | Shock Therapy | Treat one failed die as a success. |
| King | Mercury Pills | After rolling, reroll any number of dice once. |
Multiple specials on one patient all activate. Cards are revealed during surgery.

Tiebreaker: fewest dead → most patients claimed → shared victory.
Every lunatic has a type (shown on the card). Types matter for secret objectives:
Lunatic • Melancholic • Hysteric • Possessed • Maniac
★ Famous People also have a type and score bonus points that scale with how many you cure. Patients with a pair symbol grant +2 bonus if you cure both halves.
Revealed at end of game. Each rewards a different playstyle:

| The Director | +1 for every 3 patients cured |
| The Researcher | +2 for each pair of same-type patients cured |
